It's def a North American thing that I've noticed. I've had to ask for water with no ice here and in Canada, and when I ask for warm water I ALWAYS get befuddled looks. I've also had to ask for "tea without the tea" in order to get a glass of warm-to-hot water. By default all restos here serve cold water WITH ICE (as if it being cold wasn't bad enough).
